How to register with usTo register at the practice you will need to complete the requisite application form as well as a health questionnaire. These both need to be submitted to the reception. You will then need to attend a new patient registration health check. Your details will be sent to the Primary Care Trust who will issue your new medical card. Please note, all children registering with the practice who are under age of 5 will need to submit an immunisation record as a compulsory part of the registration. Although your registration will be with one particular doctor, you can see any GP of your choice. We can register you if you live within the practice area shown below: How do I make an appointment?Appointments for the doctors are available throughout the day while those for other members of the primary health care team vary throughout the week. Appointments may be made by telephoning the surgery or by using our online service . They can be booked up to three weeks in advance which provides patients with a wider choice. If you can't keep your appointment please tell us as soon as possible. How do I arrange an urgent appointment?Patients requesting an appointment on the same day or advice can speak to one of our nurses or a doctor who will assess their condition over the telephone. What to do when the surgery is closed?In the case of a medical emergency outside of normal surgery hours please either call the usual surgery number where a recorded message will advise you how to contact the duty doctor or ring the out of hours service on 0845 60 80 320. Out-of-hours cover is provided by Northern Doctors Urgent Care. You can also contact NHS Direct on 0845 46 47. This is a nurse-led service that can advise you on self management or advise you to contact your GP or, in certain circumstances to go directly to the Accident & Emergency department of your local hospital. How do I obtain a home visit?If for medical reasons you are unable to attend the surgery please call before 10.00am if possible. The doctor will call you back to discuss your request. Remember that it will often be quicker for you to come along to the surgery. How do I obtain test results?Please call the surgery between 2.00-4.00pm Monday to Friday to obtain the results of your investigations. You should telephone approximately one week following the test. The GP will have already advised the reception staff of your results, so these can usually be given to you directly over the telephone. On occasion you may be asked to discuss the results with your doctor either over the telephone or by making an appointment. This is usually because the laboratory results are complicated to translate and therefore better explained by your doctor. You will be contacted whenever you have a test result that requires further treatment. During the first seven days ask your employer for a self certificate of sickness absence. If the doctor anticipates that you will need to be absent from work for seven or more days he/she will issue you with Fit Note which will satisfy social security and statutory sick pay criteria. If you have been absent from work for less than seven days and your employer still insists on a sick note the doctor may issue a private note for which a fee will be charged. What happens if I move outside the practice area?Patients are advised to inform the practice as soon as possible when moving away permanently. After you notify us of your move we will inform the Health Authority and your records will be returned .They will then ensure that your records are moved to your new GP. Remember: Not informing us that you have moved will result in a delayed transfer of your records. How do I see a hospital specialist?If a patient wishes to see a hospital specialist they must first be assessed by a doctor. Once a referral is made, a letter would be sent out within a week of the consultation. Waiting times for appointments vary depending on the hospital or clinic. What should I do if I have not heard within 3-6 weeks after being referred?If you had not received any correspondence from the hospital or clinic, you are advised to contact the practice. If you have received a password and reference number on choose and book, please contact the hospital or clinic to which the referral was made. Who should have a flu Jab?For most people, flu is nasty but usually not serious. The vaccine is offered to at-risk groups in an effort to combat serious complications of flu.
